Everyday challenges can significantly impact a person’s health. For example, if an individual experiences unstable housing, lacks reliable transportation to medical appointments, or struggles with proper nutrition, existing health conditions may worsen—even when medical care is available. To help individuals overcome these challenges, UNC launched its Community Health Department in 2014 to serve as a bridge between healthcare providers and community support services, with the goal of improving overall health outcomes.

To help individuals better manage their healthcare needs, a team of Community Health Navigators provides personalized support to connect individuals with medical resources and healthcare services. Through these efforts, navigators work directly with clients to assist them in navigating healthcare options and understanding their treatment plans. These services are available at no cost to participants and have proven effective in helping individuals take control of their health. Navigators also lead health education efforts, community outreach, and screening events in the region.

To support a more comprehensive approach to health, the team also includes licensed clinical social workers who provide mental health support and counseling. Individual counseling appointments are available for a variety of concerns, including stress, anxiety, grief, life transitions, and substance use challenges. In addition to therapy, social workers help clients connect with additional support services, such as food, housing, rehabilitation, support groups, or medication management.

Another important focus of the Community Health team is delivering evidence-based programs that support long-term health management. One such program is the Chronic Disease Self-Management Program (CDSMP), which provides peer-led support for individuals managing chronic health conditions. Other programs focus on arthritis, diabetes, smoking cessation, and chronic pain.
